If UPROOTED is equipped to meet your needs, you will be sent an invitation to your new patient portal.

Prior to starting, you will be asked to have a full physical. You may get this at your primary doctor’s office, specialist or urgent care. We can help with finding a location if needed.

You may be asked to have screening test completed prior to starting the program.

You will start your program with Dr. Lazarus.

After 6 months, you will be invited to a maintenance program to maintain your progress.

The art of functional medicine is not adequately supported by health insurance companies or government sponsored programs. We do not accept Medicare or Medicaid or insurance of any kind.

Our doctors do not provide primary care or urgent care services. Our physicians will personally design health plans providing individualized care that give you information to help address the root cause of your health concerns, complementary to the vital care that your primary care physician and specialists provide.

Uprooted offers a 6-month program - tailored to each individual patient -(One first virtual visit, going over medical history, questionnaires, pre-requisite exams and imaging, followed by one review of findings virtual visit, three follow up visits, unlimited electronic communication, one unscheduled virtual visit (if needed), access to the patient portal, activity, nutrition, and supplement recommendations, in addition to an prescription medication sent to the pharmacy).
